If you aren't a Veela then you were turned and generally, those people have names. There are certain things you must know and note down. It doesn't matter whether your character doesn't know who these people are, they still have identities. The only area I'll let slide is 'other' because that is not compulsory.

How/Why your character was turned: Alexis was walking around the gorund, after being unable to sleep, and Kyle had just transformed. She was pacing around the Great Lake when she heard a howl from behind her. It was Kyle(but she didn't know). As he stepped closer to her she drew her wand from her robes and pointed it at him. But before she could say any spell he jumped on her and pinned her to the ground. No matter how much she struggled she couldn't get free and Kyle turned her. Now all she remembers is waking up in the Hospital Wing the next morning.
